$85/session for each followup session
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions about physical therapy coverage:
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you cancel within 24 hours of your appointment, you will be charged $25. If you miss your appointment without notifying the clinic, it will be considered a "no show", and you will be charged $50. You, not your insurance, will be responsible for payment. Since we do not allow overbooking on our schedule, it is imperative that we do not let any portion of the day go unused! Please be courteous to those that may be on the waiting list, and give us as much time as possible to notify them of your upcoming absence.
